Complex Care Manager:
  • Complex Care Manager will work collaboratively to provide Telephonic, Office-based Case Management to high risk, all age clients.
  • Read, interpret and apply clinical information, practice guidelines and analyze data and other information.
  • Create a care plan for high-risk patients and manage complex, chronic conditions and coordinate social as well as other services.
  • Conditions including arthritis, asthma, diabetes, hypertension, heart disease, COPD, HIV, osteoporosis, mental health, substance abuse and others.
  • Communicate effectively with diverse groups of people both internally as well as externally.
  • Exhibit high level of organization, time management and ability to problem-solve.
  • Administer protocols for utilization review, discharge planning and/or Case Management.
  • In-depth knowledge of health care delivery systems as they apply to Case Management.
Lead Care Manager:
  • Lead Care Manager provides daily oversight of the Care Management team, in collaboration with Supervisor & Manager of Care Management.
  • Goals include consistent work processes and compliance with NCAQ and regulatory standards.
  • Lead Case Manager will oversee team workflow including Referrals, chronic and complex Case Management and Acuity for all age patients (pediatric through geriatric).
  • Carry caseload as assigned.
  • Assist with training and staff development.
  • Utilizes clinical knowledge to apply evidence- based practice guidelines and criteria.
  • Conducts and records both focused and standard QI audits in collaboration with Supervisor.
  • Tracks and trends performance, identifies root causes, and reports and recommends improvement opportunities to management, based on audit results.
  • Coaches, trains, and conducts follow up for team performance improvement.
  • Collaboration with the supervisor to identify, design and deliver training modules to promote improved performance and/or implement new regulatory guidelines.
  • 5+ years of RN experience in acute care (ICU, ER, MedSurg, Telemetry) or similar clinical setting.
  • 2+ years of Case Management, Discharge Planning, Admissions, Utilization or similar.
  • Leadership experience preferred.
  • Managed Care experience highly preferred.
  • CCM (Certified Case Manager) highly preferred.
  • Positive and professional with strong organizational and communication skills.
