RN Telephonic Case Manager

 Approximately 10 calls/day, Mon - Fri. Age patients: Adults and Geriatrics. Patient Population: Recently Discharged patients and annual assessments. Physician Support & Interaction: via Health Connect/electronic health record. Communication Process: Completed assessments and narrative summaries to be forwarded; order requests. Ancillary Support and Services: Pharmacy. RN will not float. Will need to be open to learn how to navigate internal KP and external community based resources. Medication system: Medication reconciliation completed telephonically. Concerns to be escalated to pharmacist as needed, who is housed in same unit. Documentation System: Electronic health record / Health Connect HC). 3 types of questionnaires to be imported and utilized. All questionnaires to be followed by a narrative summary in SOAP or SBAR format, and will be routed to PCP. Personalized mailings will also be concurrently completed and sent to patient.
