CCS Case Manager/RN Specailist

 Reporting to the Director or Manager of  Case Management. A Registered Nurse, the CCS Nurse Specialist/Case Manager coordinates the care and services of assigned patient populations across the continuum of illness; promotes and monitors effective utilization of health care resources; and assumes a leadership role with the multi-disciplinary care team to achieve optimal clinical and resource outcomes. In designated Special Care Centers, provides Team Centered Care in a collaborative setting in accordance with the standards and regulations for California Children's Services (CCS). Will need to have Pediatric Cardiology experience and or Case Management and ICU experience.  This is a  cross continuum role, both inpatient and outpatient management.

•    Minimum of five years of nursing in pediatric acute care or ambulatory setting; previous experience in case management, care coordination or discharge planning role.
•    At least one year clinical experience or demonstrated clinical expertise in diagnostic group or specialty area; previous management experience (charge nurse or educator acceptable).
•    Bachelor's degree in health related field or equivalent experience.
•    BSN or master's preparation.
•    Certification in Case Management.
•    Leadership, communication, conflict resolution, cultural sensitivity.
•    Able to work autonomously and be accountable for work, with attention to detail.
•    Demonstrates effective written and verbal communication skills; critical thinking and problem solving skills; ability to resolve conflicts, including situations where there may be conflicting priorities.
•    Excellent interpersonal skills regarding the Associates, patients, visitors, etc. Intermediate computer skills, including use of e-mail and experience in use of MS Office Word and Excel.
•    Knowledge of CCS/MediCal regulations and requirements. Understanding of third party payor methods and regulations.
•    Previous experience leading process improvement or change initiatives.
•    Program development experience.
