EXPERIENCE AND SKILLS REQUIRED
Reporting to the Vice President of CHOC Health Alliance, provides coordination of the CHOC Care Guidelines Program and provides organization wide leadership of Performance Improvements Knowledge of quality and process improvement principals and tool. Statics, data collection, analysis and data presentation. Knowledge of care pathways, clinical guild lines. Ability to set performance goals and analyze results. Computer literacy and ability to learn new software programs and databases.
ESSENTIAL JOB FUNCTIONS
This person will not be in the hospital. This positions is going to to the medical clinics in the community to improve practices. The main goal of this positions is to reduce practice variations using Care guidelines
They will need to supervise 4 other RN quality specialists
the perfect person will come out a managed service or IPA like Memorial Care, St Joseph health systems, Carmore. Monarch, AltaMed
Will need to analyze processes and practices to implement quality initiatives
Provides leadership for assists in development, implements and monitor Care Guidelines in collaboration with Medical Director of Population Health and the Care Guidelines Committee. Facilitates development and reports quality metrics ( in association with the Care Guidelines Development team) by which to monitor the outcomes, effectiveness and compliance with the use of Care Guidelines. Assures data accuracy and utilizes sound statistical principal's tools in reporting and analysis for ambulatory practices. Champion's evidence based medicine/practices through the development and implementation of Care Guidelines. Works Collaboratively with the Medical Director of population Health in the development of revision of processes within the electronic medical record that support the use of Care Guidelines. Provides data analysis of CHOC's and Ambulatory practices high volume and high risk diagnoses and procedures to Care Guidelines Committee that will provide basis for development of Care Guidelines. . Develops and implements communication and education strategies for new /revised Care Guidelines and polices and procedures. Actively communicates/educates about new and revised Care Guidelines updates studies an references and new education tools to appropriate clinical groups. Serves as resource to staff and physicians regarding the Care Guidelines Program. Performs oversight and training of Performance Improvement Teams lead by Quality Improvement Advisors and deployed. Regularly assess the progress of the PI team and practices they are assigned too. Assists leaders and facilitators with tools, reports and presentations as needed to stay on track and accomplish the goals of the teams. Plans and conducts provider assessments and trainings. Develops appropriate deployment strategy for QI Advisors across the community. Monitors The Division of population Heath high level results across the community of pediatricians, identifies gaps in performance, and makes recommendations for improvement. Regularly reviews The Division of Population Health's high level scorecards and other reports ( with Exception of Financial performance) and identifies gaps in performance, report recommendations for improvement to the Medical Director of Population health. Service as resource and recommends performance goals , incremental targets and thresholds based on historical performance, comparative data and best practice. Responsible for specifics HEDIS and Appropriate Resource Use Metrics as well as oversight of the CMMIU Grant measure targeted at Bronchiolitis, communicate Acquired Pneumonias, Asthma, Acne, Headaches and Gastroenteritis care and resource use. Service as the Division of Populations Health's expert/resource for Performance Improvements. Educates the organization as needed and service as an expert/resource for PI approaches including methodology (i.e. PDCA0 Data analysis and assessment's and measurement tools such as flow charts, Pareto charts, and statistical process control charts. Establishes and maintain on line ( intranet) Performance Improvement resources for the Organization.
Bachelor of Science Nursing Degree MHA/MBA
Section D: Skill Requirement(s)*
5 years Clinical Experience designing and implementing Performance Improvements initiatives in a Clinical and non-clinical area in both the inpatient and outpatient settings. . 3+ years of acute care experience.
