JOB PURPOSE
The Nurse Practitioner (NP) for concussion clinic works in collaboration with the appropriate physicians and other members of the professional health care team to provide continuity of care for patients experiencing, or at risk for, a concussion. The NP will be responsible for managing a wide range of outpatient clinical duties, while adhering to standard practices, established procedures, and ensuring the quality and safe delivery of

care. He/she will evaluate, treat, and manage concussion patients in the clinic setting, while maintaining and establishing relationships with the broader community.
The NP will be instrumental in building relationships with partner organizations to expand the concussion program to meet the needs of the growing Tri-Counties region.
This position reports to the Director of Pediatric Clinics and is under physician supervision.
MAJOR ACCOUNTABILITIES / SPECIFIC JOB COMPETENCIES

Clinical Practice Functions:
Works closely with appropriate physicians and health care team members to care for patients and their families. Reviews lab tests, radiology studies and other pertinent data to determine interventions in collaboration with physicians.
Performs neurological assessments according to evidenced-based practice and formulates appropriate plan of care.
Evaluates patient readiness to return to normal activity following a head injury, discussing goals and parameters with multidisciplinary team as needed.
Provides detailed and updated information to patients and their families in regards to clinical course and ongoing treatment plan.
Refers patient to appropriate services based on examination findings and plan of care.

Coordination and Collaboration Functions:
Collaborates with the multidisciplinary team regarding all aspects of care for the patients.
Facilitates Physician consultations when necessary and implements their recommendations after discussion with the team.
Maintains and builds relationships with county schools and other agencies in the advancement of the concussion program.
Assists with development and implementation of clinical practice guidelines and standardized evidenced- based supportive tools.
Demonstrates effective time management to promote productivity and cost efficiency.
Uses material goods wisely and is accountable for prudent use of our talents and financial resources.
Works collaboratively between departments to provide continuity of care.

Patient Education Functions:
Provides education to patients and families about plan of care.
Promotes patient and family self-care through education and support. Counsels patients and families and encourages optimal physical, social and psychological adjustment.
Provides supportive interventions and referrals to families when appropriate.
Develops and maintains necessary education materials, including establishing policies and procedures as needed.

Research Functions:
Promotes scientific inquiry in clinical practice, using the research and evidenced-based practices to provide care.
Evaluates published research for application into clinical practice. Disseminates information about recent innovations and research findings relevant to nursing practice and patient outcomes to other health care team members.
Assists other staff in the interpretation and application of research findings to clinical practice.

Education Functions
Maintains professional competence by attending local and national conferences and educational seminars. Shares information from conferences with other members of the healthcare staff. Participates in Magnet activities to share education.
Participates in the education of other members of the health care team.
Facilitates clinical experiences and participates in the mentorship and preceptorship of APP students and clinical staff as appropriate.

With variations due to hospital site and department, this position may require census-driven scheduling flexibility (additional/fewer shifts), holiday coverage, floating, being placed on-call, weekend coverage, and shift rotations.

QUALIFICATIONS

LEVEL OF EDUCATION
Minimum:
Master of Nursing Degree required
Graduate of an accredited Nurse Practitioner program approved by the California Board of Registry as defined in Section 1482, Nurse practitioner Act, Business and professional Code.
Preferred:
CERTIFICATIONS, LICENSES, REGISTRATIONS
Minimum:
Current State of California Registered Nurse license
Current California license to practice as a Nurse Practitioner issued by the California State board
Valid Drug Enforcement Agency license
Approval of privileges by CHS Medical Staff department
