Care Connector

The Care Connector is responsible for supporting the daily operations of integrated care management and utilization management program interventions. The Care Connector performs in a contact center environment, effectively processing calls from Members, Providers and other areas, internal and external to the company.
The Care Connector will be responsible for:
• Processing inbound telephone calls
• Conducting outbound calls
• Data entry/processing
The Care Connectors identifies members with Care Gaps/HEDIS related health conditions and assists them in accessing care through Plan benefits and community resources. Under the direction of Clinical staff, the Care Connector will provide members with educational materials and carry out strategies to increase health care adherence and reduce barriers to care.
The assignment will be 3 to 6 months temporary employment. The hours for training will be Monday through Friday 8:30 am to 5:00pm. The hours after training will be Monday through Friday 9:30 am to 6:00pm.
The ideal candidate will be self-directed and self-motivated.
• High School/GED Required. Associates preferred.
• One to three years of experience in Health Care, Medicaid knowledge a plus.
• Customer Service experience in a call center or contact center environment preferred.
• Experience with Microsoft Office a plus, specifically Excel and Word.
