Description:Facilitates department workflow by supporting clinical team with various care coordination workflows. Locates community services and other referral locations for enrollees, arranges access to care, supports communication with hospitals, and providers.

- Provides non-clinical support to workflows involving utilization management, case management, disease management, and health promotion. Trouble shoots for the clinical team when an appropriate provider, type of service, or appropriate level of care cannot be readily found. Researches availability of community services in order to link members to appropriate services. Assists in transitions in care for hospitalized patients and makes appropriate placements as needed.
- Assumes responsibility for self-development and career progression. Proactively seeks and participates in ongoing training (formal and informal) in all aspects of the Care Worker role. Assists in the mentoring and training of new Care Workers. Remains responsible for updating self on ever changing information to ensure accuracy when dealing with members and providers.

An Associate Degree is required

The selected candidate will assist members with Behavioral Health issues, who are out on Worker Comp, to return to worker sooner because of the services like coaching, assessments, screenings and on-going evaluations provided by Magellan. The selected candidate is the first point of contact and will complete a basic screening. To be successful in this role, the candidate must be able to engage with the enrollee and convey the benefits of this program in an energetic manner to the enrollee. The candidate will also schedule initial and follow up calls for Coaches using outlook. The candidate must be computer literate, organized, a quick learner and flexible. The hours of operation for this program may vary; however, at this time they are from 7am – 7pm. The candidate will work 8 hours within that time period with a tentative start between 8am and 9am. Understanding of plan benefit structures, psychiatric/medical terminology, and local communication resources
